I get the same situation here.
It seems that OpenFTA can handle 2 AND ports. As soon as I add a third AND port, it crashes !

The bug still continue happening in the version at this date 20 -Jan-2015
As I see there will be no support or future correction of this..
What a shame, anyway thanks for develop this nice tool

Still reproduces in the latest available version for Windows. Is there any chance to fix it?

What causes this is a bit of a mystery.
Copying the directory with all the files in it together to another blank directory can cause it to crash. any changes to fault tree risk Monte carlo Crashing. I am now stuck as I dont want to corrupt the files I have working but I also want to try some "what if" exercises with thier contents.

This occurs if the top level gate is an AND but not if it is an OR!

Note this occurs with a simple 2 leaf branch with a single and condition. If the and is set to an Or the problem goes away.

Open FTA closes when Monte carlo Simulation is run
When I run Monte Carlo Simulation on my fault Tree the progress bar is shown for a while but then it closes and closes OpenFTA. The Monte carlo Report produced is incomplete as below"
